Guidelines for Completing the Student Application for the Hearing Committees
  • There are two applications for completion.  If you wish to be considered for either the Chair position only or the Chair and Member positions, please complete the "Chair-Member Application".  If you wish only to be considered for the Member position, please complete the "Member Application".
  • For recommendations, please include two (2) completed Recommendation Forms (print off twice).  These should be professional (not personal) references, including professors, advisors, former/current supervisors, mentors, etc.  Please do not use relatives or friends.
  • Students must bring to Costo 104 a signed copy in order to have their application be considered.
  • Only students need to complete the application in order to become a member of SCC or CAIC.
  • Students have to be approved by ASUCR (undergraduates) or GSA (graduates).
